Eligibility Criteria:
- Requirements for Clients: Clients must have a diagnosed disability, mental health condition, or health-related need that requires long-term care.
- Income, Residency, and Other Criteria: Clients must be residents of Minnesota, and income eligibility follows state guidelines for waiver services.
- Exceptions: Unique cases, such as emergency needs or complex medical situations, may be considered on an individual basis.
