Job Summary
We are seeking a compassionate and dedicated Case Manager to join our team at Frontier Living. In this role, you will be responsible for providing support and guidance to individuals and families in need, helping them navigate various social services and achieve stability in their lives.
Key Responsibilities
- Assess client needs and develop individualized care plans
- Coordinate and monitor the delivery of services to clients
- Conduct regular check-ins and home visits with clients
- Maintain accurate and up-to-date case records
- Collaborate with other service providers and community resources
- Advocate for clients' rights and access to services
- Participate in team meetings and case conferences
Qualifications
- Bachelor's degree in Social Work, Psychology, or related field
- 2+ years of experience in case management or social services
- Strong interpersonal and communication skills
- Ability to work with diverse populations
- Knowledge of local community resources and social service systems
- Proficiency in Microsoft Office and case management software
- Valid driver's license and reliable transportation
